To see a world in a Grain of Sand,
And a Heaven in a Wild Flower,
Hold Infinity in the palm of your hand,
And eternity in an hour.

I hope I understood the question correctly: Why was the lamp made once with a patterned glass and once with a plain glass?
Simply because first Alchemy couldn't get the patterned glass, so made them without, and when they found someone who could make the pattern, they made the lamp with patterned glass.
All such variations on one product are due to availability of the materials used.
That's the reason you can find different items with different colored crystals or slightly different shape/color of glass used in the glassware etc.
It's simply due to the availability of materials. If they can't get one particular material, they will substitute it with something similar if they can.

Ah, didn't notice it before, was only paying attention to the glass.
Have no real idea. Probably the design team thought about the addition later, so they just changed the design after they already produced some of the plain ones.

Looks to me like someone had a plain Alchemy dish and attached a coffin with a skeleton to it. The coffin part doesn't look to me like it's from Alchemy, but it is really similar in design. Can't see the trademarks from the coffin really well, but they don't look like Alchemy trademarks to me.
